- Develop and implement a comprehensive strategy to address social determinants of health within the organization.
- Collaborate with cross-functional teams to identify and address health disparities and inequities within communities.
- Conduct research and analysis to identify key social determinants of health and their impact on health outcomes.
- Design and implement initiatives and programs to address social, economic, and environmental factors that influence health.
- Develop partnerships and collaborations with community organizations, government agencies, and other stakeholders to support the organization's social determinants of health efforts.
- Monitor and evaluate the effectiveness of social determinants of health initiatives and make recommendations for improvement.
- Lead and manage a team of professionals to ensure successful implementation of social determinants of health strategies.
- Serve as a subject matter expert on social determinants of health, providing guidance and support to other departments and stakeholders.
- Stay up-to-date on the latest research, trends, and best practices in the field of social determinants of health.
- Represent the organization at conferences, events, and other public forums to promote the organization's social determinants of health initiatives and raise awareness of health disparities and inequities.
- Collaborate with marketing and communications teams to develop messaging and materials to promote social determinants of health initiatives and engage members and the community.
- Ensure compliance with all relevant laws, regulations, and ethical standards related to social determinants of health.
- Develop and manage budgets, grants, and contracts related to social determinants of health initiatives.
- Foster a culture of health equity and diversity within the organization.
Advanced Degree In Public Health, Social Work, Or A Related Field With A Focus On Social Determinants Of Health.
Minimum Of 7-10 Years Of Experience In A Leadership Role, Preferably In A Healthcare Or Public Health Setting.
Demonstrated Expertise In Developing And Implementing Strategies To Address Social Determinants Of Health, Including Community Engagement, Partnerships, And Health Equity Initiatives.
Strong Understanding Of Healthcare Systems And Policies, As Well As Social, Economic, And Environmental Factors That Impact Health Outcomes.
Excellent Communication And Collaboration Skills, With The Ability To Work Effectively With Diverse Stakeholders, Including Community Organizations, Government Agencies, And Healthcare Providers.
