Are you a motivated and compassionate Registered Nurse with experience in complex pediatric cases?   1. Coordinate and manage complex pediatric cases by collaborating with the patient, their family, and healthcare professionals to develop and implement effective care plans.
  2. Conduct in-depth assessments of patients' medical conditions, treatment plans, and progress to identify their specific needs and ensure they receive appropriate care.
  3. Provide regular communication and support to patients and their families, answering any questions and addressing concerns they may have.
  4. Utilize critical thinking skills to identify potential barriers to care and develop solutions to improve the patient's overall well-being.
  5. Monitor and track patient progress, documenting any changes or updates in their condition and updating care plans accordingly.
  6. Educate patients and their families on their medical conditions, treatment options, and resources available to them.
  7. Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care for patients.
  8. Stay up-to-date on the latest advancements and treatments in pediatric healthcare to provide the best care possible.
  9. Adhere to all legal and ethical standards, ensuring patient confidentiality and privacy at all times.
  10. Participate in team meetings and trainings to share knowledge and contribute to the continuous improvement of our complex case management program.
  11. Work independently and remotely while maintaining a high level of productivity and meeting established metrics.
  12. Show compassion and empathy towards patients and their families, providing emotional support and advocating for their needs.
  13. Adhere to company policies and procedures, as well as state and federal regulations, to ensure quality and compliant care.
  14. Handle any urgent or emergency situations with professionalism and efficiency, ensuring the safety and well-being of patients at all times.
  15. Collaborate with insurance companies and other payers to ensure appropriate coverage and reimbursement for patients' care.

Job Qualifications
  • Minimum Of 3 Years Of Experience In Pediatric Case Management: Candidates For This Position Should Have A Strong Background And Understanding Of Case Management In A Pediatric Setting. This Includes Experience Working With Children And Families, Coordinating Care Plans, And Navigating Complex Medical Systems.

  • Active Rn License In The State Of Employment: As A Complex Case Manager Rn, Candidates Must Hold An Active And Unrestricted Registered Nurse License In The State In Which They Will Be Working. This Ensures That The Candidate Is Qualified And Authorized To Provide Nursing Care And Case Management Services.

  • Knowledge Of Pediatric Medical Conditions And Treatments: The Ideal Candidate For This Position Should Possess A Deep Understanding Of Pediatric Medical Conditions, Treatments, And Procedures. This Includes Knowledge Of Common Childhood Illnesses, Chronic Diseases, And Specialized Pediatric Interventions.

  • Excellent Communication And Interpersonal Skills: A Complex Case Manager Rn Must Be Able To Effectively Communicate With Patients, Families, And Healthcare Providers. This Requires Strong Written And Verbal Communication Skills, As Well As The Ability To Build Rapport And Maintain Relationships With Diverse Individuals.

  • Experience With Remote Case Management: Due To The Remote Nature Of This Position, Candidates Should Have Prior Experience With Telephonic And/Or Virtual Case Management. This Includes The Ability To Effectively Assess And Manage Patient Needs And Provide Support And Guidance Remotely. Knowledge Of Telehealth Technology And Electronic Medical Records Is Also Preferred.
