- Provide specialized support and services to individuals with complex behavioral health needs.
- Collaborate with interdisciplinary team members to develop and implement individualized treatment plans.
- Conduct assessments and evaluations to identify needs and develop appropriate interventions.
- Monitor and evaluate progress towards treatment goals and make adjustments as needed.
- Utilize evidence-based practices and techniques to support individuals in achieving positive outcomes.
- Provide crisis intervention and de-escalation techniques when necessary.
- Maintain accurate and timely documentation of all interactions and interventions.
- Advocate for individuals and their families to ensure access to necessary resources and services.
- Stay up-to-date on current trends and research in the field of behavioral health.
- Participate in ongoing training and professional development opportunities.
- Serve as a liaison between clients, families, and community resources.
- Collaborate with community organizations to promote awareness and understanding of behavioral health issues.
- Adhere to ethical and legal standards in all interactions and interventions.
- Maintain confidentiality and respect the privacy of individuals and their families.
- Provide support and guidance to other team members as needed.
Minimum Of A Bachelor's Degree In Behavioral Health Or Related Field
At Least 3-5 Years Of Experience Working In A Behavioral Health Setting, Preferably With High Acuity Patients
Knowledge And Understanding Of Evidence-Based Practices For Treating Behavioral Health Conditions
Strong Crisis Intervention And De-Escalation Skills
Ability To Work Independently And Remotely, While Also Collaborating With A Multidisciplinary Team To Provide Comprehensive Care For Patients.
