- Provide emotional support and counseling to patients and families during end-of-life journeys.
- Conduct comprehensive assessments of patients' psychosocial needs and develop individualized care plans.
- Collaborate with interdisciplinary team members to ensure holistic care for each patient.
- Connect patients and families with appropriate community resources and support services.
- Advocate for patients' rights and facilitate communication between patients, families, and healthcare providers.
- Provide education and guidance to patients and families on end-of-life care options and advance care planning.
- Monitor and address any changes in patients' mental health and well-being.
- Document all interactions and interventions accurately and in a timely manner.
- Stay up-to-date on hospice care policies and procedures, as well as relevant social work practices and theories.
- Maintain confidentiality and adhere to ethical standards in all interactions with patients and families.
- Participate in team meetings, trainings, and other professional development opportunities.
- Support and contribute to a positive and collaborative work environment within the hospice department.
- Continuously evaluate and improve the quality of care provided to patients and families.
- Foster a culture of compassion, empathy, and dignity for patients and families.
- Serve as a liaison between the hospice department and other healthcare providers and community organizations.
Master's Degree In Social Work From An Accredited Program: A Social Worker-Hospice At Henry Ford Health System Must Possess A Master Of Social Work (Msw) Degree From An Accredited Program. This Ensures That The Candidate Has Received Comprehensive Training In Areas Such As Counseling, Case Management, And Advocacy, Which Are Essential Skills For A Hospice Social Worker.
Relevant Experience In Hospice Or Palliative Care: The Ideal Candidate Should Have Previous Experience Working In A Hospice Or Palliative Care Setting. This Experience Will Demonstrate The Candidate's Understanding Of The Unique Needs And Challenges Of Terminally Ill Patients And Their Families.
Licensure As A Licensed Master Social Worker (Lmsw): In Addition To An Msw Degree, The Candidate Must Hold A Current Lmsw License. This License Is Required To Practice Social Work In The State Of Michigan And Ensures That The Candidate Has Met The Necessary Educational And Ethical Standards.
Strong Communication And Interpersonal Skills: A Social Worker-Hospice At Henry Ford Health System Must Possess Excellent Communication And Interpersonal Skills. They Will Work Closely With Patients, Families, And Interdisciplinary Teams, And Must Be Able To Effectively Communicate And Collaborate With All Parties Involved.
Knowledge Of Community Resources And Support Services: The Candidate Should Have A Strong Understanding Of Community Resources And Support Services Available To Hospice Patients And Their Families.
