- Provide compassionate end-of-life care to hospice patients in their homes.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Monitor and assess patients' physical and emotional well-being, making adjustments to care plans as needed.
- Administer medications and treatments as prescribed by physicians.
- Maintain accurate and timely documentation of patient care.
- Educate patients and their families on managing symptoms and providing emotional support.
- Communicate effectively with patients, families, and team members.
- Ensure patient comfort and dignity throughout the end-of-life process.
- Adhere to all legal and ethical standards in patient care.
- Participate in ongoing professional development and training to maintain current knowledge and skills.
- Maintain a safe and clean environment for patients and their families.
- Serve as an advocate for patients and their families, ensuring their needs and wishes are respected.
- Demonstrate empathy and sensitivity towards patients and their families during difficult times.
- Collaborate with community resources and organizations to provide comprehensive care.
- Perform other duties as assigned by the Hospice Home Care Manager.
Current Rn License In The State Of Employment.
Minimum Of 2 Years Of Experience In Hospice Or Home Care Setting.
Knowledge Of Medicare/Medicaid Regulations And Guidelines Related To Hospice Care.
Strong Communication And Interpersonal Skills To Effectively Collaborate With Interdisciplinary Team And Patients' Families.
Ability To Provide Compassionate And End-Of-Life Care To Patients With Terminal Illnesses.
