Job Description

  1. Provide compassionate and dedicated care to hospice patients in the acute care setting.
  2. Oversee and coordinate the care of hospice patients in collaboration with interdisciplinary team members.
  3. Utilize clinical expertise to assess patients' needs and create individualized care plans.
  4. Collaborate with physicians and other healthcare professionals to ensure effective and timely treatment.
  5. Educate patients and their families on hospice care and provide emotional support during difficult times.
  6. Monitor patient progress and make necessary adjustments to treatment plans.
  7. Ensure compliance with all regulatory and organizational policies and procedures.
  8. Maintain accurate and thorough documentation of patient care.
  9. Serve as a resource to other healthcare professionals and provide guidance on hospice care.
  10. Continuously evaluate and improve processes to enhance the quality of care provided to patients.
  11. Actively participate in team meetings and provide input on patient care and organizational initiatives.
  12. Stay up-to-date with industry trends and advancements in hospice care.
  13. Demonstrate a commitment to providing exceptional care and improving outcomes for patients.
  14. Collaborate with community resources and organizations to enhance the hospice care experience for patients and their families.
  15. Maintain a professional and respectful demeanor in all interactions with patients, families, and team members.

Job Qualifications
  • Bachelor Of Science In Nursing (Bsn) Degree From An Accredited Institution

  • Active Registered Nurse (Rn) License In The State Of Michigan

  • Minimum Of 2 Years Of Experience In Hospice Care And/Or Acute Care Setting

  • Certification In Hospice And Palliative Care Nursing (Chpn) Preferred

  • Strong Interpersonal And Communication Skills, With The Ability To Collaborate Effectively With Interdisciplinary Teams And Provide Compassionate Care To Patients And Their Families.
