- Assist in developing and implementing individualized physical therapy plans to meet the specific needs of patients in the home health setting.
- Provide direct patient care under the supervision of a licensed physical therapist, including exercises, modalities, and other therapeutic interventions.
- Monitor and document patient progress, including any changes in condition or response to treatment.
- Instruct and educate patients and their families on exercises and techniques to improve mobility, strength, and function.
- Communicate effectively with the physical therapy team, including therapists and other assistants, to ensure coordinated and comprehensive care for patients.
- Maintain accurate and organized patient records, including treatment plans, progress notes, and discharge summaries.
- Adhere to all safety and infection control policies and procedures to ensure a safe and healthy environment for patients and staff.
- Continuously expand knowledge and skills through continuing education and training opportunities.
- Collaborate with other healthcare professionals, such as nurses and social workers, to provide holistic and patient-centered care.

- Follow all state and federal regulations and laws related to physical therapy practice and home health care.

- Maintain confidentiality of patient information and adhere to all HIPAA regulations.
- Flexibility to travel to patient homes and adapt to varying schedules and environments.

Associate's Degree In Physical Therapy Assistant Program From An Accredited Institution.
Valid State Licensure Or Certification As A Physical Therapy Assistant.
Minimum Of 1-2 Years Of Experience In A Home Health Care Setting.
Knowledge Of And Ability To Apply Physical Therapy Techniques And Modalities To Treat Patients With Various Conditions.
Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With Patients, Caregivers, And Other Healthcare Professionals.
