We are currently seeking a skilled and compassionate Physical Therapist to join our team in our Home Health Care department.   1. Provide exceptional care to patients in their homes by conducting thorough evaluations, developing treatment plans, and implementing evidence-based interventions.
  2. Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive and coordinated care for patients.
  3. Maintain accurate and timely documentation of patient progress, treatment plans, and other necessary information.
  4. Educate patients and their families on proper techniques for exercises and activities to improve their condition and prevent further injury.
  5. Continuously monitor and reassess patient progress, adjusting treatment plans as needed.
  6. Adhere to all ethical and legal standards, as well as policies and procedures of Henry Ford Health System.
  7. Maintain a safe and clean working environment, following infection control and safety protocols.
  8. Act as an advocate for patients, ensuring their rights and preferences are respected and considered in the treatment process.
  9. Keep up-to-date with advancements in physical therapy techniques and technology, and incorporate them into practice when appropriate.
  10. Actively participate in team meetings, trainings, and other professional development opportunities.
  11. Communicate effectively with patients, families, and colleagues to establish and maintain positive relationships.
  12. Adhere to all regulatory and compliance requirements, including maintaining appropriate licensure and certifications.
  13. Demonstrate a commitment to providing compassionate and patient-centered care.
  14. Perform other related duties as assigned by supervisor.

Job Qualifications
  • Bachelor's Or Master's Degree In Physical Therapy From An Accredited Institution.

  • Strong Communication And Interpersonal Skills To Effectively Work With Patients And Their Families In A Home Setting.

  • Active And Unrestricted State License To Practice Physical Therapy.

  • Minimum Of 2 Years Of Experience In Home Health Care Or Related Field.

  • Cpr Certification And Knowledge Of Basic Life Support Techniques.
