  1. Conduct specialized medication therapy management for patients with complex hematologic and oncologic conditions.
  2. Collaborate with the interdisciplinary team to develop and implement individualized treatment plans for patients.
  3. Provide education and counseling to patients about their medications, possible side effects, and potential drug interactions.
  4. Monitor patients' medication therapy and make necessary adjustments to ensure optimal outcomes.
  5. Stay up-to-date with new medications and treatment options in the field of hematology/oncology.
  6. Maintain accurate and detailed patient records.
  7. Work closely with physicians and other healthcare professionals to ensure seamless care for patients.
  8. Communicate effectively with patients, caregivers, and other healthcare team members.
  9. Participate in quality improvement initiatives to enhance patient care.
  10. Adhere to all relevant laws, regulations, and policies related to medication management and patient safety.
  11. Attend and actively contribute to team meetings and professional development activities.
  12. Provide guidance and support to pharmacy technicians and other staff members.
  13. Demonstrate a commitment to providing compassionate and patient-centered care.
  14. Maintain a high level of professionalism and ethical standards in all interactions.
  15. Continuously evaluate and improve processes to enhance efficiency and patient outcomes.

Job Qualifications
  • Bachelor's Degree In Pharmacy Or Doctor Of Pharmacy (Pharmd) From An Accredited Institution.

  • Completion Of A Residency Program In Hematology/Oncology Pharmacy Or Equivalent Experience In A Clinical Setting.

  • Active Licensure As A Pharmacist In The State Of Michigan.

  • Board Certification In Oncology Pharmacy (Bcop) Preferred.

  • Experience Working With Complex Chemotherapy Regimens And Knowledge Of Hematology/Oncology Medications And Treatment Protocols.
