  4. Demonstrate a strong passion for hospice care.
  5. Possess necessary qualifications and skills as a Nurse Practitioner.
  6. Serve as a leader and role model for other healthcare professionals.
  7. Ensure compliance with all healthcare regulations and protocols.
  8. Develop and implement protocols and procedures to ensure efficient and effective care delivery.
  9. Conduct regular performance evaluations and provide feedback and coaching to team members.

  11. Participate in the recruitment, hiring, and training of new team members.
  12. Monitor and maintain accurate patient records and documentation.
  13. Collaborate with other healthcare professionals to develop and implement patient care plans.
  14. Stay updated on current hospice care practices and guidelines.
  15. Communicate effectively with patients and their families to address any concerns or questions.
  16. Continuously strive to improve patient outcomes and satisfaction.
  17. Participate in quality improvement initiatives and assist in developing strategies for improvement.
  18. Represent the hospice team at meetings and events within the healthcare system.
  19. Maintain a positive and professional attitude at all times.
  20. Serve as a supportive and compassionate presence for patients and their families during end-of-life care.

Job Qualifications
  • Master's Degree In Nursing (Msn) From An Accredited Nurse Practitioner Program

  • Active And Unrestricted Advanced Practice Registered Nurse (Aprn) And Nurse Practitioner (Np) License In The State Of Michigan

  • Minimum Of 5 Years Of Experience As A Hospice Nurse Practitioner, With At Least 2 Years In A Leadership Or Managerial Role

  • Certification As A Hospice And Palliative Care Nurse Practitioner (Achpn) From A Recognized Certifying Body (E.g. Ancc Or Hpcc)

  • Strong Clinical And Interpersonal Skills, With The Ability To Effectively Manage And Lead A Team Of Nurses And Other Healthcare Professionals In A Fast-Paced Hospice Setting.
