The Henry Ford Health System is currently seeking an experienced and passionate Integrated Case Manager - Population Health to join our care team. In this role, you will be responsible for providing comprehensive care coordination services to members of our Population Health program. You will work with members to develop and implement individualized care plans, coordinate services, and help them achieve their health goals.To be successful in this role, you must bring a combination of strong clinical and interpersonal skills. You should have an in-depth understanding of the healthcare system, and a commitment to quality patient care. You must also possess the ability to provide culturally competent care, and have the ability to build trusting partnerships with members and their families.
