  10. Adhere to ethical and professional standards of practice while advocating for patient rights and autonomy.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Nursing, Or Related Field

  • Minimum Of 2-3 Years Of Experience In Case Management, Preferably In A Healthcare Setting

  • Knowledge Of Healthcare Systems, Insurance, And Community Resources

  • Strong Communication And Interpersonal Skills, With The Ability To Build Relationships And Work Collaboratively With Interdisciplinary Teams

  • Demonstrated Ability To Assess, Plan, Implement, And Evaluate Care Plans For Patients With Complex Medical And Social Needs.
