  1. Develop and implement individualized care plans for patients in collaboration with a multidisciplinary team.
  2. Advocate for patients' needs and work towards achieving their health and wellness goals.
  3. Manage complex cases and ensure coordination of care across various healthcare settings.
  4. Assess patient needs and provide appropriate referrals and resources.
  5. Communicate and collaborate with patients, families, and healthcare providers to ensure effective and efficient care.
  6. Monitor and evaluate patient progress and adjust care plans as needed.
  7. Educate patients and their families about their health conditions and treatment plans.
  8. Document patient interactions, care plans, and progress in a timely and accurate manner.
  9. Stay up-to-date on healthcare policies and regulations related to case management.
  10. Participate in care conferences and meetings to discuss patient progress and develop strategies for improving care.
  11. Serve as a liaison between patients, healthcare providers, and insurance companies to ensure appropriate coverage and reimbursement.
  12. Maintain confidentiality and adhere to ethical and legal standards in all aspects of the job.
  13. Collaborate with community resources and organizations to provide comprehensive care for patients.
  14. Participate in quality improvement initiatives and provide feedback for process improvement.
  15. Continuously seek opportunities for professional development and growth to enhance case management skills.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Nursing, Psychology, Or Related Field.

  • Minimum Of 2-3 Years Of Experience In Case Management, Care Coordination, Or Related Field.

  • Knowledge Of Healthcare Systems, Insurance Coverage, And Resources For Patients.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Healthcare Team, And Community Resources.

  • Ability To Multitask, Prioritize, And Problem Solve In A Fast-Paced And Dynamic Environment.
