  1. Build and maintain strong relationships with members of the community, including patients, families, and community organizations.
  2. Conduct outreach and engage with individuals and families in the community to assess their health needs and provide education and resources.
  3. Collaborate with healthcare providers and other members of the healthcare team to develop and implement plans for improving community health.
  4. Provide support and guidance to patients and their families in navigating the healthcare system, including scheduling appointments, obtaining insurance coverage, and accessing resources.
  5. Conduct home visits to assess living conditions and provide education on healthy living practices.
  6. Develop and facilitate community health education programs on various topics such as nutrition, chronic disease management, and preventive care.
  7. Advocate for the needs of the community and work towards improving access to healthcare services.
  8. Maintain accurate and detailed records of community interactions and services provided.
  9. Continuously evaluate and adapt community health programs to meet the changing needs of the community.
  10. Participate in community events and meetings to promote community health initiatives and foster relationships with community members.
  11. Stay updated on current health trends and resources in the community to provide the most relevant and effective support to community members.
  12. Collaborate with other community health workers and organizations to share best practices and resources.
  13. Adhere to all ethical and confidentiality guidelines when working with community members.
  14. Continuously strive to improve the health and well-being of the community through innovative and compassionate approaches.

Job Qualifications
  • Bachelor's Degree In Public Health, Community Health, Social Work, Or Related Field.

  • Minimum Of 2 Years Of Experience In Community Outreach, Health Education, Or Related Field.

  • Knowledge Of Community Resources And Ability To Connect Individuals To Appropriate Services.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Engage With Diverse Populations.
