As a Care Manager, you will play a crucial role in coordinating and managing the care of our patients, ensuring that they receive the highest quality of care.   1. Coordinate and manage the care of patients within the Henry Ford Health System, ensuring that they receive comprehensive and compassionate care.
  2. Implement and adhere to all policies and procedures related to patient care, in alignment with the mission and values of the organization.
  3. Collaborate with healthcare team members, including physicians, nurses, and other care providers, to develop and implement individualized care plans for patients.
  4. Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.
  5. Conduct assessments of patient needs and develop appropriate interventions to address those needs.
  6. Monitor patient progress and make necessary adjustments to care plans to ensure optimal outcomes.
  7. Educate patients and their families on available resources and support services within the community.
  8. Advocate for patients and their families to ensure they receive the highest quality of care.
  9. Document all patient interactions and care plans accurately and in a timely manner.
  10. Continuously evaluate and improve care management processes to enhance patient satisfaction and outcomes.
  11. Stay up-to-date on industry trends and best practices in care management, and incorporate them into daily practices.
  12. Adhere to all ethical and legal standards of care and maintain patient confidentiality at all times.
  13. Participate in interdisciplinary team meetings and communicate effectively with team members to ensure coordinated and efficient care.
  14. Act as a role model for the organization's values and mission, promoting a culture of compassion and excellence in patient care.
  15. Collaborate with other departments within the organization to ensure seamless patient care and services.
  16. Provide guidance and mentorship to junior care management staff members, as needed.
  17. Adhere to all regulatory and accreditation standards related to patient care and documentation.
  18. Maintain accurate and up-to-date patient records and ensure compliance with all medical record documentation requirements.
  19. Participate in performance improvement activities and contribute to the development of new care management initiatives.

Job Qualifications
