- Conduct comprehensive assessments of patients' social, emotional, and environmental needs
- Develop individualized treatment plans in collaboration with patients, families, and healthcare team members
- Provide counseling and support to patients and families facing difficult medical decisions or challenges
- Coordinate with other healthcare professionals to ensure holistic and coordinated care for patients
- Advocate for patients' rights and access to resources and services
- Maintain accurate and up-to-date documentation of patient interactions and progress
- Participate in interdisciplinary team meetings and provide input on patient care plans
- Collaborate with community organizations and agencies to facilitate patient referrals and access to resources
- Continuously update knowledge on social work practices and techniques through professional development opportunities
- Adhere to ethical standards and maintain confidentiality in all patient interactions.
Master's Degree In Social Work (Msw Or Mssw) From An Accredited University.
State Licensure As A Licensed Clinical Social Worker (Lcsw) Or Licensed Master Social Worker (Lmsw).
Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In A Hospital Or Medical Facility.
Experience And Knowledge Of Social Work Theories, Assessments, And Interventions For A Variety Of Populations And Conditions.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team And Effectively Advocate For Patients' Needs.
