  1. Act as a liaison between patients and rehabilitation services, ensuring that patients receive the best care possible.
  2. Assess patients' rehabilitation needs and connect them with appropriate services within the organization.
  3. Collaborate with physicians, therapists, and other healthcare professionals to develop comprehensive rehabilitation plans for patients.
  4. Educate patients and their families on available rehabilitation services and answer any questions or concerns they may have.
  5. Maintain accurate and up-to-date patient records and documentation.
  6. Communicate effectively with patients, families, and healthcare team members to ensure seamless coordination of care.
  7. Continuously evaluate and monitor patients' progress and make necessary adjustments to their rehabilitation plans.
  8. Stay updated on current rehabilitation practices and procedures to provide the highest quality of care.
  9. Adhere to all organizational policies, procedures, and standards of care.
  10. Uphold the organization's mission, vision, and values in all interactions with patients and team members.
  11. Participate in team meetings and case conferences to discuss patient care and progress.
  12. Collaborate with marketing and outreach teams to promote rehabilitation services and increase patient referrals.
  13. Maintain confidentiality of patient information in accordance with HIPAA regulations.
  14. Foster a positive and professional work environment and contribute to a culture of teamwork and collaboration.
  15. Maintain a flexible schedule to accommodate patient needs and organizational demands.

Job Qualifications
  • Excellent Communication And Interpersonal Skills

  • Bachelor's Degree In Nursing, Physical Therapy, Occupational Therapy, Or Related Field

  • Minimum Of 2 Years Clinical Experience In A Healthcare Setting

  • Knowledge Of Rehabilitation Services And Ability To Assess Patient Needs
