- Act as a liaison between patients and rehabilitation services, ensuring that patients receive the best care possible.
- Assess patients' rehabilitation needs and connect them with appropriate services within the organization.
- Collaborate with physicians, therapists, and other healthcare professionals to develop comprehensive rehabilitation plans for patients.
- Educate patients and their families on available rehabilitation services and answer any questions or concerns they may have.
- Maintain accurate and up-to-date patient records and documentation.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless coordination of care.
- Continuously evaluate and monitor patients' progress and make necessary adjustments to their rehabilitation plans.
- Stay updated on current rehabilitation practices and procedures to provide the highest quality of care.
- Adhere to all organizational policies, procedures, and standards of care.
- Uphold the organization's mission, vision, and values in all interactions with patients and team members.
- Participate in team meetings and case conferences to discuss patient care and progress.
- Collaborate with marketing and outreach teams to promote rehabilitation services and increase patient referrals.
- Maintain confidentiality of patient information in accordance with HIPAA regulations.

- Maintain a flexible schedule to accommodate patient needs and organizational demands.
Excellent Communication And Interpersonal Skills
Bachelor's Degree In Nursing, Physical Therapy, Occupational Therapy, Or Related Field
Minimum Of 2 Years Clinical Experience In A Healthcare Setting
Knowledge Of Rehabilitation Services And Ability To Assess Patient Needs
Ability To Work Independently And As Part Of A Multidisciplinary Team.
