  1. Provide exceptional pharmaceutical care to patients by accurately reviewing and dispensing medications, monitoring drug therapy and providing medication education and counseling.
  2. Collaborate with healthcare team members to develop and implement treatment plans that optimize patient outcomes.
  3. Ensure compliance with all federal, state and local laws and regulations related to pharmacy practice.
  4. Maintain accurate and complete patient medication records.
  5. Serve as a drug information resource for healthcare team members and patients.
  6. Contribute to the development and implementation of clinical pharmacy programs and initiatives.
  7. Participate in quality improvement efforts to enhance patient care and safety.
  8. Stay up-to-date on the latest developments and advancements in pharmaceutical products and treatments.
  9. Foster positive relationships with patients, caregivers, and other healthcare professionals.
  10. Maintain confidentiality of patient information and adhere to ethical standards.
  11. Complete all required documentation accurately and in a timely manner.
  12. Supervise and train pharmacy technicians and other support staff.
  13. Participate in multidisciplinary rounds and provide recommendations for medication therapy.
  14. Communicate effectively with patients and healthcare team members to provide optimal care.

  16. Actively participate in continuing education and maintain licensure and certifications.
  17. Maintain a safe and clean work environment.
  18. Perform other duties as assigned by supervisor.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy.

  • Board Certification As A Pharmacotherapy Specialist (Bcps) Or Other Relevant Specialty Certification.

  • Minimum Of 3-5 Years Of Experience Practicing As A Clinical Pharmacist In A Hospital Or Healthcare Setting.

  • Strong Knowledge Of Pharmacotherapy Principles, Medication Management, And Patient Care Processes.

  • Excellent Communication And Teaching Skills, With The Ability To Collaborate With Multidisciplinary Teams And Educate Patients On Medication Use And Management.
