  1. Provide administrative support to the case management team, including scheduling appointments, maintaining records, and organizing documents.
  2. Assist with coordinating patient care services, such as arranging transportation and coordinating appointments with healthcare providers.
  3. Communicate effectively with patients, families, and healthcare providers to ensure a smooth and efficient case management process.
  4. Collaborate with the case management team to develop and implement care plans for patients, ensuring that their needs are met and their goals are achieved.
  5. Maintain confidentiality and adhere to HIPAA regulations when handling sensitive patient information.
  6. Monitor and track patient progress and outcomes, and provide regular updates to the case management team.
  7. Conduct research and gather information to assist in decision-making and problem-solving for patient care.
  8. Identify and report any potential issues or concerns to the case management team, ensuring that patients receive the best possible care.
  9. Participate in team meetings and provide input on ways to improve the case management process and patient care.
  10. Uphold the values and mission of HCA Healthcare by providing exceptional care to patients and supporting the team members.

  • Excellent Communication Skills: A Case Management Assistant Must Be Able To Effectively Communicate With Patients, Healthcare Providers, And Insurance Companies To Coordinate Care And Services.

  • Knowledge Of Medical Terminology And Procedures: A Strong Understanding Of Medical Terminology And Procedures Is Essential For A Case Management Assistant To Accurately Document And Track Patient Information.
