Responsibilities:

1. Provide social work services and support to patients and families in accordance with the standards of professional practice, organizational policies, and established regulatory requirements.
2. Develop and implement individualized treatment plans in collaboration with patients and their families.
3. Utilize a variety of assessment tools to identify needs and develop treatment plans.
4. Provide counseling and/or psychotherapy services as needed.
5. Monitor and document progress of patients and adjust treatment plans as needed.
6. Maintain up-to-date records of patients’ treatments and progress.
7. Consult with other healthcare professionals to coordinate care and ensure proper support and medical treatments.
8. Participate in educational and training activities to stay abreast of the latest trends and developments in the field of social work.
9. Advocate for patients’ rights and ensure their quality of life.
10. Participate in professional organizations and committees to advance the profession.
