Social Worker

Responsibilities:

1. Assess the needs of patients and their families to determine appropriate care plans.
2. Provide counseling and therapeutic services to patients and their families.
3. Develop and implement social service programs in conjunction with other healthcare professionals.
4. Monitor patient progress and make adjustments to care plans as needed.
5. Maintain accurate records of patient care and progress.
6. Work with relevant agencies and organizations to secure resources and support services for patients.
7. Develop and maintain relationships with community partners to enhance patient care.
8. Adhere to relevant regulations and professional standards of practice.
9. Provide education and training to patients and their families on available resources.
10. Identify and address any ethical or legal issues in patient care.
