Responsibilities: • Work closely with interdisciplinary team to provide comprehensive services to residents and their families. • Serve as a resource to the interdisciplinary team in regards to psychosocial issues. • Provide individual and group counseling services to residents and families. • Maintain complete, accurate, and timely documentation of all services provided. • Monitor services and identify areas for improvement. • Develop and implement social service policies and procedures. • Administer the social service program, including intakes, assessments, and care plans. • Ensure compliance with applicable state and federal regulations. • Provide education and training to staff on social service topics. • Participate in discharge planning and aftercare services. • Collaborate with outside agencies to coordinate resources for residents. • Participate in multidisciplinary team meetings.
