MDS Coordinator - RN

In this role, you will be responsible for coordinating and overseeing the Minimum Data Set (MDS) process for our facility, ensuring accurate and timely completion of assessments and care plans for our residents. 1. Coordinate and oversee the Minimum Data Set (MDS) process for the facility.
2. Ensure accurate and timely completion of assessments and care plans for residents.
3. Work closely with interdisciplinary team to gather necessary information for MDS assessments.
4. Utilize knowledge of MDS rules and regulations to ensure compliance.
5. Maintain up-to-date knowledge of changes in MDS guidelines and communicate updates to team.
6. Collaborate with nursing staff to ensure accurate and complete documentation for MDS assessments.
7. Review and analyze resident medical records to ensure accuracy and completeness.
8. Participate in interdisciplinary care conferences to develop and update individualized care plans.
9. Communicate with physicians and other healthcare providers to gather necessary information for MDS assessments.
10. Train and educate staff on MDS guidelines and documentation requirements.
11. Monitor and track resident care plans to ensure appropriate interventions are documented and implemented.
12. Provide support and guidance to team members regarding MDS assessments and care plans.
13. Maintain confidentiality of resident information and adhere to HIPAA regulations.
14. Participate in quality improvement initiatives to enhance resident care and outcomes.
15. Uphold the mission and values of Genesis HealthCare in all interactions and responsibilities.
