  1. Conducting thorough and accurate assessments of patients' medical conditions and needs through the completion of the Minimum Data Set (MDS) assessment.
  2. Coordinating and collaborating with other healthcare professionals to develop individualized care plans for patients.
  3. Ensuring compliance with all state and federal regulations for MDS assessments.
  4. Maintaining and updating patient records and documentation in a timely and organized manner.
  5. Monitoring and evaluating patient progress and making necessary adjustments to care plans.
  6. Communicating effectively with patients, families, and healthcare team members to provide quality care and support.
  7. Continuously staying informed on new developments and changes in MDS protocols and regulations.
  8. Assisting with training and mentoring new staff members on MDS assessment procedures.
  9. Maintaining confidentiality and adhering to ethical standards in all aspects of the job.
  10. Collaborating with other departments to ensure accurate and timely billing and reimbursement for services provided.
  11. Participating in quality improvement initiatives and implementing best practices to improve patient outcomes.
  12. Maintaining a safe and clean work environment for patients and staff.
  13. Ensuring all patient care is provided in accordance with facility policies and procedures.
  14. Demonstrating a commitment to providing compassionate and patient-centered care.
  15. Representing the facility in a professional and positive manner at all times.

Job Qualifications
  • Valid Rn License: A Mds Coordinator - Rn Must Hold A Current And Valid Registered Nurse License In Their State Of Practice.

  • Minimum Of 2 Years Clinical Experience: Genesis Healthcare Requires A Minimum Of 2 Years Of Experience As A Registered Nurse In A Long-Term Care Or Post-Acute Care Setting.

  • Certification In Mds: Candidates Should Have A Strong Understanding Of The Minimum Data Set (Mds) And Be Certified In Mds By The American Association Of Nurse Assessment Coordination (Aanac).

  • Knowledge Of Medicare And Medicaid Regulations: The Mds Coordinator - Rn Must Possess A Thorough Understanding Of The Regulations And Guidelines Governing Medicare And Medicaid Reimbursement For Skilled Nursing Facilities.

  • Strong Communication And Leadership Skills: The Mds Coordinator - Rn Must Have Excellent Communication Skills And The Ability To Lead And Collaborate With A Multidisciplinary Team, Including Physicians, Therapists, And Other Healthcare Professionals.
