- Coordinate and manage the Minimum Data Set (MDS) process, ensuring accuracy and compliance with regulatory guidelines.
- Utilize clinical knowledge and expertise to assess and evaluate residents' care needs and develop individualized care plans.
- Collaborate with interdisciplinary team members to ensure comprehensive and holistic care for residents.
- Monitor and document changes in residents' conditions, and update care plans accordingly.
- Communicate effectively with residents, families, and healthcare team members regarding care plans and resident progress.
- Provide education and training to staff on MDS process and regulations.
- Participate in quality improvement initiatives to enhance resident care and outcomes.
- Maintain up-to-date knowledge of industry trends and regulations related to MDS.
- Ensure timely submission of MDS assessments and documentation to regulatory bodies.
- Promote a positive and supportive work environment that fosters teamwork and professional growth.
- Assist in the recruitment and training of MDS staff members.
- Adhere to all company policies, procedures, and standards of practice.
- Collaborate with external agencies and professionals to coordinate resident care and services.
- Ensure confidentiality and privacy of resident information.
- Represent the facility in a professional and positive manner at all times.
