
MDS Coordinator

As an MDS Coordinator, you will play a crucial role in ensuring accurate and timely completion of resident assessments, maintaining compliance with state and federal regulations, and collaborating with interdisciplinary teams to develop individualized care plans. - Conduct thorough resident assessments to determine their individual care needs and develop personalized care plans.
- Ensure timely and accurate completion of Minimum Data Set (MDS) assessments in compliance with state and federal regulations.
- Collaborate with interdisciplinary teams, including physicians, nurses, and therapists, to develop and implement effective care plans.
- Monitor and update resident care plans as needed, taking into consideration changes in health status and needs.
- Ensure all documentation is complete, accurate, and in compliance with regulatory guidelines.
- Communicate assessment findings and care plans to appropriate team members and family members.
- Maintain a thorough understanding of state and federal regulations related to resident assessments and care planning.
- Participate in quality improvement initiatives to enhance resident care and outcomes.
- Provide education and training to staff regarding resident assessments and care planning.
- Participate in interdisciplinary team meetings to discuss and review resident care plans and make necessary adjustments.
- Keep up-to-date with changes in regulations and best practices related to resident assessments and care planning.
- Maintain confidentiality of resident information and adhere to HIPAA guidelines.
- Establish and maintain positive relationships with residents, families, and other healthcare professionals.
- Demonstrate a compassionate and caring attitude towards residents and their families.

Bachelor's Degree In Nursing Or Healthcare Administration: A Strong Educational Background In Nursing Or Healthcare Administration Is Essential For A Mds Coordinator Role, As It Provides The Necessary Foundation In Patient Care And Administrative Functions.
Minimum Of 2 Years Experience In Long-Term Care: The Mds Coordinator Is Responsible For Overseeing The Assessment And Care Planning Process For Long-Term Care Patients, So A Minimum Of 2 Years Experience In This Setting Is Typically Required.
Knowledge Of Mds Coding And Reimbursement: The Mds Coordinator Must Have A Thorough Understanding Of Mds Coding And Reimbursement Processes, As They Are Responsible For Ensuring Accurate Documentation For Reimbursement Purposes.
Strong Organizational And Communication Skills: As The Mds Coordinator Is Responsible For Coordinating With Multiple Departments And Ensuring Accurate And Timely Assessments, Strong Organizational And Communication Skills Are Crucial For Success In This Role.
Attention To Detail And Ability To Work Independently: The Mds Coordinator Must Have A Keen Eye For Detail And Be Able To Work Independently To Complete And Submit Assessments Accurately And On Time. This Requires A High Level Of Accountability And Self-Motivation.
