1. Develop and implement social service programs that align with the mission and values of Genesis HealthCare.
2. Oversee and manage the daily operations of the social services department, including budgeting, staffing, and resource allocation.
3. Provide leadership and guidance to social services staff, including training, mentoring, and performance evaluations.
4. Collaborate with other departments and healthcare professionals to ensure a comprehensive and coordinated approach to patient care.
5. Conduct assessments and develop care plans for patients and residents, taking into consideration their physical, emotional, and social needs.
6. Monitor and evaluate the effectiveness of social service programs and make necessary adjustments to ensure quality care is provided.
7. Maintain accurate and up-to-date records and documentation in compliance with regulatory standards.
8. Act as a liaison between patients, families, and community resources to coordinate additional support services as needed.
9. Stay informed about current developments and best practices in social work and integrate this knowledge into practice.
10. Adhere to ethical standards and maintain confidentiality of patient information.
11. Participate in interdisciplinary team meetings and contribute to the development of individualized care plans.
12. Collaborate with marketing and admissions teams to promote and market social service programs and services.
13. Ensure compliance with all federal, state, and local regulations related to social services.
14. Handle any escalated issues or concerns related to social services and provide timely and effective resolution.
15. Continuously seek opportunities to improve the quality of care and services provided by the social services department.
