Director - Social Services

Genesis HealthCare is seeking an exceptional and experienced leader to join our team as Director - Social Services. As the Director - Social Services, you will be responsible for providing direct social services to our patients and families to ensure quality of life and optimal functioning in accordance with our core values.The ideal candidate will have a passion for providing social services to patients and their families and will have a minimum of a Master’s degree in Social Work from an accredited college or university. At least five years of experience in a social services setting is also required, as well as a current state certification or license and experience in geriatric social work. Responsibilities:

1. Provide direct social services to patients and families in accordance with the organization’s core values.
2. Maintain a current state certification or license in geriatric social work.
3. Develop and maintain relationships with patients, families, and other healthcare professionals.
4. Utilize excellent communication skills, both verbal and written, to effectively communicate with patients, families, and other healthcare professionals.
5. Collaborate with other healthcare professionals and teams to ensure optimal functioning of patients.
6. Monitor and evaluate patient care and progress to ensure quality of life.
7. Participate in educational activities and seminars to stay updated on the latest developments in social work.
8. Maintain accurate records of patient care.
9. Develop and implement policies and procedures to ensure compliance with applicable laws and regulations.
10. Perform other duties as assigned.
