Director - Social Services

We are looking for an experienced and compassionate Director of Social Services to join our team at Genesis HealthCare. The ideal candidate should possess exceptional organizational and interpersonal skills, and have a passion for providing excellent social services to our patients.To be successful in this role, you should have at least 3 years of experience in social services in a healthcare setting, as well as a valid Social Services certification or license. You should have a thorough knowledge of case management, discharge planning, and related regulations. Responsibilities:

1. Develop and implement comprehensive social service plans for patients and families.
2. Monitor and evaluate the effectiveness of social service plans to ensure quality care.
3. Provide counseling and support to patients and families during difficult times.
4. Develop and maintain relationships with external organizations to ensure resources are available for patients and families.
5. Participate in discharge planning meetings and provide guidance to ensure the best possible outcome for patients.
6. Track and document all patient interactions and social services provided.
7. Develop and maintain policies and procedures related to social services.
8. Participate in training and development activities to stay up-to-date with best practices.
9. Maintain a high level of professionalism and customer service when interacting with patients and families.
10. Ensure compliance with all applicable laws and regulations related to social services.
