Job Description

As a Case Manager, you will play a crucial role in coordinating and managing patient care plans, ensuring that our patients receive the best possible treatment and support.   1. Develop and implement patient care plans in collaboration with the healthcare team.
  2. Coordinate with physicians, nurses, and other healthcare professionals to ensure timely and appropriate treatment for patients.
  3. Conduct patient assessments and make recommendations for necessary interventions.
  4. Monitor and evaluate patient progress and adjust care plans as needed.
  5. Educate patients and their families on treatment plans, medications, and self-care techniques.
  6. Ensure accurate and timely documentation of patient care in electronic medical records.
  7. Communicate regularly with patients and their families to provide updates on treatment plans and address any concerns or questions.
  8. Collaborate with community resources to provide additional support services for patients.
  9. Advocate for patients and their needs within the healthcare system.
  10. Stay up-to-date with current healthcare practices and regulations.
  11. Participate in quality improvement initiatives to enhance patient care.
  12. Mentor and provide guidance to other healthcare team members.
  13. Adhere to all company policies and procedures, including safety and infection control protocols.
  14. Maintain confidentiality of patient information.
  15. Uphold the mission and values of Genesis HealthCare in all interactions with patients, families, and colleagues.

Job Qualifications
  • Active Lpn License: The Candidate Must Possess A Valid And Active Licensed Practical Nurse (Lpn) License In The State Where They Will Be Working.

  • Prior Experience In Case Management: Previous Experience In Case Management Or Care Coordination, Preferably In A Long-Term Care Or Rehabilitation Setting, Is Required For This Role.

  • Strong Communication Skills: Excellent Communication Skills, Both Verbal And Written, Are Essential For Effective Collaboration With Patients, Families, And Other Healthcare Professionals.

  • Knowledge Of Medicare And Medicaid Regulations: A Thorough Understanding Of Medicare And Medicaid Regulations And Their Impact On Patient Care Is Necessary For This Role.

  • Ability To Work Independently And As Part Of A Team: The Candidate Should Be Able To Work Independently To Manage Their Caseload, While Also Being Able To Collaborate With Other Healthcare Professionals To Provide Comprehensive Patient Care.
