Case Manager LPN

- Develop and implement patient care plans in collaboration with the healthcare team.
- Coordinate with physicians, nurses, and other healthcare professionals to ensure timely and appropriate treatment for patients.
- Conduct patient assessments and make recommendations for necessary interventions.
- Monitor and evaluate patient progress and adjust care plans as needed.
- Educate patients and their families on treatment plans, medications, and self-care techniques.
- Ensure accurate and timely documentation of patient care in electronic medical records.
- Communicate regularly with patients and their families to provide updates on treatment plans and address any concerns or questions.
- Collaborate with community resources to provide additional support services for patients.
- Advocate for patients and their needs within the healthcare system.
- Stay up-to-date with current healthcare practices and regulations.
- Participate in quality improvement initiatives to enhance patient care.
- Mentor and provide guidance to other healthcare team members.
- Adhere to all company policies and procedures, including safety and infection control protocols.
- Maintain confidentiality of patient information.

Active Lpn License: The Candidate Must Possess A Valid And Active Licensed Practical Nurse (Lpn) License In The State Where They Will Be Working.
Prior Experience In Case Management: Previous Experience In Case Management Or Care Coordination, Preferably In A Long-Term Care Or Rehabilitation Setting, Is Required For This Role.
Strong Communication Skills: Excellent Communication Skills, Both Verbal And Written, Are Essential For Effective Collaboration With Patients, Families, And Other Healthcare Professionals.
Knowledge Of Medicare And Medicaid Regulations: A Thorough Understanding Of Medicare And Medicaid Regulations And Their Impact On Patient Care Is Necessary For This Role.
Ability To Work Independently And As Part Of A Team: The Candidate Should Be Able To Work Independently To Manage Their Caseload, While Also Being Able To Collaborate With Other Healthcare Professionals To Provide Comprehensive Patient Care.
