1. Collaborate with the social services team to develop and implement strategies for improving the lives of patients and residents.

3. Assist in connecting patients and residents with necessary resources and services to meet their physical, emotional, and social needs.
4. Coordinate with other departments, such as nursing and therapy, to ensure holistic care for patients and residents.
5. Maintain accurate and up-to-date documentation of patient and resident progress and interactions.
6. Act as a liaison between patients, families, and community resources to facilitate access to necessary services.
7. Provide emotional support and counseling to patients, residents, and their families.
8. Conduct regular check-ins with patients and residents to monitor progress and address any concerns or issues.
9. Participate in team meetings and training sessions to stay informed about best practices and new developments in the field of social services.
10. Comply with all healthcare regulations, policies, and procedures to ensure the highest level of care for patients and residents.
