3. Collaborate with the Social Services team to develop and implement care plans for residents.
4. Assist residents in navigating community resources and services.
5. Advocate for residents to ensure their rights and needs are met.
6. Conduct regular assessments to identify changes in residents' needs and update care plans accordingly.
7. Maintain accurate and confidential records of residents' care and progress.
8. Participate in interdisciplinary team meetings to discuss residents' progress and concerns.
9. Provide emotional support and guidance to residents and their families.
10. Educate residents and their families on available resources and support services.
11. Assist with discharge planning and ensure smooth transitions for residents.
12. Stay up-to-date on relevant laws, regulations, and resources related to social services.
13. Collaborate with external agencies and organizations to provide the best possible care for residents.
14. Continuously evaluate and improve the effectiveness of social services provided.
15. Adhere to all company policies and procedures, as well as ethical and professional standards.
