- Educate and inform patients and their families about different sleep disorders, their causes, symptoms, and treatment options.
- Conduct patient assessments to determine the most appropriate treatment plan based on their specific needs and medical history.
- Collaborate with physicians, sleep technologists, and other healthcare professionals to develop and implement individualized treatment plans for patients.
- Provide guidance and support to patients throughout their sleep journey, including scheduling appointments, answering questions, and addressing any concerns.
- Monitor patient progress and make necessary adjustments to treatment plans as needed.
- Educate patients on the proper use of sleep equipment, such as CPAP machines, and provide ongoing support and troubleshooting assistance.
- Maintain accurate and up-to-date patient records, including medical history, test results, and treatment plans.
- Stay current on developments and advancements in sleep medicine and therapies through ongoing training and education.
- Provide education and training to other healthcare professionals on sleep disorders and treatments.
- Maintain a compassionate and empathetic approach when working with patients and their families, understanding the impact of sleep disorders on their overall well-being.
- Adhere to all ethical and legal standards and guidelines in the delivery of care to patients.
- Participate in quality improvement initiatives to enhance the effectiveness and efficiency of the sleep program.
- Collaborate with insurance companies to obtain authorization for treatments and ensure proper billing and reimbursement.
- Maintain confidentiality and adhere to HIPAA regulations in handling patient information.
- Serve as a liaison between patients and healthcare providers, ensuring effective communication and coordination of care.
Bachelor's Degree In Nursing Or Related Field: A Sleep Navigator At Franciscan Health Should Have A Solid Foundation In Healthcare And Nursing Principles To Effectively Navigate Patients Through The Sleep Program.
Certification In Sleep Medicine: Candidates With A Certification In Sleep Medicine, Such As The Registered Polysomnographic Technologist (Rpsgt), Will Have A Deep Understanding Of Sleep Disorders And Treatment Options, Making Them Well-Equipped For This Role.
Experience In Patient Education And Counseling: As A Sleep Navigator, One Must Be Skilled In Educating And Counseling Patients On Sleep Disorders, Treatment Plans, And Lifestyle Modifications. Previous Experience In Patient Education Or Counseling Is Highly Desirable.
Strong Communication And Interpersonal Skills: Effective Communication Is Crucial For A Sleep Navigator To Build Trust And Rapport With Patients, As Well As Collaborate With A Multidisciplinary Team Of Healthcare Professionals.
Organizational And Time-Management Abilities: The Role Of A Sleep Navigator Involves Managing A Caseload Of Patients, Coordinating Appointments, And Ensuring Timely Follow-Up. Strong Organizational And Time-Management Skills Are Essential To Succeed In This Role.
