If so, Franciscan Health is seeking a Medication Reconciliation Technician to join our team! As a Medication Reconciliation Technician, you will play a crucial role in our patients' health by reconciling medication orders and identifying any discrepancies to ensure accurate and appropriate treatment. We are looking for dedicated individuals with a background in pharmacy or healthcare, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team.   1. Conduct medication reconciliation for patients admitted to the hospital, ensuring accuracy and completeness of medication orders.
  2. Identify and resolve any discrepancies in medication orders, working closely with the healthcare team to ensure safe and effective treatment for patients.
  3. Document all medication reconciliation activities accurately and timely in patient records.
  4. Communicate with patients and their families regarding medication changes and provide education on proper medication use.
  5. Collaborate with pharmacists, nurses, and other healthcare professionals to ensure seamless medication management for patients.
  6. Review patient histories and current medication lists to identify potential drug interactions or allergies.
  7. Follow all safety protocols and procedures to prevent medication errors and promote patient safety.
  8. Stay updated on new medications and changes in medication guidelines to maintain knowledge and competency.
  9. Participate in quality improvement initiatives related to medication reconciliation and patient safety.
  10. Maintain confidentiality and adhere to all HIPAA regulations in handling patient information.
  11. Attend trainings and meetings as required by the organization.
  12. Provide excellent customer service to patients and their families, addressing any concerns or questions related to medication reconciliation.
  13. Assist in maintaining inventory and ordering of medications and supplies.
  14. Follow all institutional and departmental policies and procedures.
  15. Contribute to a positive and collaborative work environment by being a team player and supporting fellow team members.

  • Certification As A Pharmacy Technician: Candidates For A Medication Reconciliation Technician Position At Franciscan Health Should Have A Valid Certification As A Pharmacy Technician From An Accredited Institution.
