
Hospice Patient Care Coordinator

- Coordinate and facilitate the care of hospice patients in collaboration with the hospice care team.
- Develop individualized care plans for each patient, taking into consideration their physical, emotional, and spiritual needs.
- Conduct initial and ongoing assessments of patients to determine their level of care and any necessary adjustments to their care plan.
- Communicate effectively with patients, families, and other healthcare professionals to ensure continuity and quality of care.
- Serve as a resource for patients and their families, providing information and support throughout the hospice care journey.
- Monitor and evaluate the effectiveness of the care plan and make necessary changes as needed.
- Ensure compliance with all regulatory requirements and standards for hospice care.
- Coordinate and schedule appointments, treatments, and consultations for patients as needed.
- Collaborate with interdisciplinary team members to provide comprehensive and holistic care to patients.
- Maintain accurate and up-to-date patient records and documentation.
- Educate patients and families on end-of-life care and provide guidance and support during difficult decision-making processes.
- Participate in interdisciplinary team meetings and contribute to the development and implementation of policies and procedures.
- Foster a compassionate and supportive environment for patients and their families, promoting a sense of dignity and respect during their end-of-life journey.
- Stay updated on current and emerging hospice care practices and techniques to continuously improve the quality of care provided to patients.
- Adhere to ethical and professional standards and maintain confidentiality of patient information.
Excellent Communication And Interpersonal Skills
Bachelor's Degree In Nursing Or Related Field
Minimum Of 2 Years Experience In Hospice Care Or Related Field
Knowledge Of Medicare And Medicaid Regulations And Hospice Reimbursement
Ability To Provide Compassionate And Quality Care To Patients And Families In A Hospice Setting.
