Job Description

Are you passionate about providing compassionate care to patients and their families during one of life's most difficult times? Franciscan Health is seeking a dedicated Hospice Intake Coordinator to join our team. In this role, you will serve as the initial point of contact for patients seeking hospice services and work closely with our interdisciplinary team to ensure a smooth transition into our care. We are looking for someone with excellent communication and organizational skills, as well as a strong understanding of hospice care. If you are a compassionate and detail-oriented individual with a desire to make a difference in the lives of others, we would love to hear from you.

  • Serve as the initial point of contact for patients seeking hospice services
  • Communicate with patients and their families in a compassionate and empathetic manner
  • Conduct initial intake assessments to determine eligibility for hospice care
  • Collaborate with our interdisciplinary team to ensure a smooth transition into hospice care
  • Provide information and resources to patients and families regarding hospice services and end-of-life care
  • Maintain accurate and organized patient records and documentation
  • Coordinate and schedule appointments and consultations for patients
  • Communicate with healthcare providers and other organizations involved in the patient's care
  • Stay up-to-date on hospice care regulations and guidelines
  • Participate in team meetings and discussions to provide input and support for patient care
  • Continuously strive to provide the highest level of care and support to patients and their families.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Social Work, Or A Related Field.

  • Minimum Of 2 Years Of Experience In Hospice Or Palliative Care Intake Coordination.

  • Knowledge Of Hospice Regulations, Medicare And Medicaid Guidelines, And Insurance Billing.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work With Patients, Families, And Healthcare Professionals.

  • Strong Organizational And Time-Management Skills, With The Ability To Prioritize Tasks And Handle Multiple Responsibilities Simultaneously.
