1. Conduct initial patient assessments and create individualized care plans for each patient in accordance with physician orders.
2. Administer medications and treatments as prescribed by the physician and document all patient interactions accurately.
3. Provide skilled nursing care, including wound care, IV therapy, and catheter management, to patients in their homes.
4. Update patient records and communicate any changes in condition to the appropriate healthcare team members.
5. Collaborate with other healthcare professionals to ensure the continuity of care and best outcomes for patients.
6. Educate patients and their families about their medical conditions, treatment plans, and self-care techniques.
7. Monitor patient progress and make adjustments to the care plan as needed.
8. Maintain a safe and clean environment for patients and adhere to infection control policies and procedures.
9. Participate in quality improvement initiatives and assist with data collection and analysis.
10. Adhere to all federal, state, and local laws and regulations, as well as company policies and procedures.
11. Attend and actively participate in team meetings, in-services, and continuing education opportunities.
12. Serve as a patient advocate and provide emotional support to patients and their families.
13. Maintain confidentiality of patient information and adhere to HIPAA regulations.
14. Travel to patients' homes as needed and maintain a reliable mode of transportation.
15. Perform other related duties as assigned by the Home Health Manager.
