As a Discharge Planner, you will play a crucial role in ensuring our patients receive the best possible care during their transition from hospital to home.   1. Develop and implement discharge plans for patients based on their individual needs and medical conditions.
  2. Collaborate with interdisciplinary teams to coordinate and facilitate a smooth transition for patients from hospital to home.
  3. Communicate with patients and their families to provide education and support regarding post-discharge care and medication management.
  4. Review medical records and consult with healthcare providers to ensure appropriate discharge planning and follow-up care.
  5. Coordinate and schedule necessary medical equipment, home health services, and follow-up appointments for patients.
  6. Advocate for patients to ensure their needs and preferences are met during the discharge process.
  7. Stay current on insurance regulations and coverage to assist patients in accessing necessary resources.
  8. Ensure accurate and timely documentation of discharge plans and communication with healthcare team members.
  9. Participate in discharge planning meetings and provide input on best practices and improvements.
  10. Maintain confidentiality and adhere to ethical standards in all aspects of the job.
  11. Collaborate with community resources to provide patients with necessary support and assistance after discharge.
  12. Continuously evaluate and adjust discharge plans to meet the changing needs of patients.
  13. Adhere to all hospital policies and procedures related to discharge planning and patient care.
  14. Provide emotional support and counseling to patients and their families during the discharge process.
  15. Attend trainings and educational opportunities to enhance skills and knowledge related to discharge planning and patient care.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or Related Field.

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In Discharge Planning.

  • Knowledge Of Medicare, Medicaid, And Private Insurance Regulations And Guidelines.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Multidisciplinary Healthcare Teams.

  • Ability To Assess Patient Needs, Develop Individualized Discharge Plans, And Coordinate Post-Discharge Services.
