  1. Provide education and support to patients with diabetes to help them better manage their condition and improve their overall health and well-being.
  2. Work closely with the healthcare team to develop individualized treatment plans for patients with diabetes.
  3. Conduct thorough assessments of patients' medical history, current health status, and lifestyle to identify specific needs and areas for improvement.
  4. Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment for patients.
  5. Utilize evidence-based practices and resources to educate patients on the latest advancements in diabetes management and self-care techniques.
  6. Monitor and evaluate patients' progress and adjust treatment plans as necessary to achieve optimal outcomes.
  7. Provide emotional support and counseling to patients and their families to help them cope with the challenges of living with diabetes.
  8. Stay up-to-date with the latest research and developments in the field of diabetes management and incorporate them into patient care.
  9. Participate in community outreach and education programs to promote diabetes awareness and prevention.
  10. Maintain accurate and thorough documentation of patient care and progress in accordance with healthcare regulations and standards.
  11. Act as a resource and mentor for other healthcare professionals, providing guidance and support in diabetes education and management.
  12. Adhere to ethical and legal standards in patient care and maintain patient confidentiality at all times.

Job Qualifications
  • Certification As A Diabetes Educator (Cde) Through The National Certification Board For Diabetes Educators (Ncbde).

  • Current Licensure As A Physician In The State Of Practice, With Board Certification In Endocrinology Or Internal Medicine Preferred.

  • Minimum Of 2 Years Of Experience As A Practicing Physician, With A Strong Background In Managing Patients With Diabetes.

  • Knowledge Of Evidence-Based Practice Guidelines For Diabetes Management, Including The American Diabetes Association Standards Of Care.

  • Strong Communication And Patient Education Skills, With The Ability To Develop Individualized Treatment Plans And Provide Ongoing Support And Education To Patients With Diabetes.
