  1. Develop and implement individualized diabetes education plans for patients, including instruction on self-care practices such as blood glucose monitoring, medication management, and healthy lifestyle habits.
  2. Conduct thorough assessments of patients' medical histories, current health status, and knowledge of diabetes management to identify areas for improvement.
  3. Collaborate with interdisciplinary healthcare teams to provide comprehensive care to patients with diabetes, including physicians, nurses, dietitians, and pharmacists.
  4. Deliver engaging and informative presentations to groups of patients, caregivers, and community members on various aspects of diabetes management, prevention, and treatment.
  5. Stay up-to-date on the latest research, trends, and best practices in diabetes education and integrate this knowledge into patient care.
  6. Utilize effective communication and counseling techniques to address patients' emotional and psychological needs related to their diabetes diagnosis and management.
  7. Develop and maintain relationships with local organizations and resources to support patients in accessing necessary services and resources for diabetes management.
  8. Monitor and track patients' progress towards achieving their diabetes management goals and make necessary adjustments to their education plan as needed.
  9. Maintain accurate and thorough documentation of patient interactions, education sessions, and progress in electronic medical records.
  10. Serve as a resource for healthcare professionals within the organization, providing guidance and support on diabetes management and education.

Job Qualifications
  • Bachelor's Degree In Nursing, Nutrition, Or A Related Field

  • Certified Diabetes Educator (Cde) Designation From The National Certification Board For Diabetes Educators (Ncbde)

  • Minimum Of 2 Years Of Experience In Diabetes Education Or Related Field

  • Knowledge Of Current Diabetes Management Guidelines And Techniques

  • Excellent Communication And Interpersonal Skills To Effectively Educate And Support Patients With Diabetes
