- Develop and implement evidence-based protocols and guidelines for the safe and effective use of anticoagulant medications.
- Collaborate with physicians and other healthcare professionals to provide comprehensive pharmaceutical care to patients receiving anticoagulation therapy.
- Conduct medication therapy management and provide drug information to healthcare team members and patients.
- Monitor patients' response to anticoagulant therapy and make appropriate adjustments to medication regimens.
- Educate patients and caregivers on the proper use and potential side effects of anticoagulant medications.
- Oversee and assist in the management of anticoagulation clinics.
- Participate in quality improvement initiatives to ensure the highest standards of care for patients receiving anticoagulation therapy.
- Stay up-to-date on the latest developments and research in anticoagulation therapy and apply this knowledge to patient care.
- Maintain accurate and thorough patient records and documentation.
- Serve as a resource and provide guidance to other healthcare professionals on anticoagulation therapy.
- Adhere to all regulatory and ethical standards in the practice of pharmacy.
- Promote a culture of patient safety and quality care within the pharmacy department and across the organization.
- Continuously evaluate and improve upon processes and procedures related to anticoagulation therapy.
- Participate in interdisciplinary rounds and provide recommendations for medication management.
- Mentor and supervise pharmacy students and residents in the area of anticoagulation therapy.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program.
Board Certification In Pharmacotherapy Or Ambulatory Care Pharmacy.
Minimum Of 2 Years Of Experience In Anticoagulation Management In A Clinical Setting.
Strong Knowledge Of Anticoagulation Medications And Their Appropriate Uses, Dosing, And Monitoring.
Excellent Communication And Interpersonal Skills To Effectively Collaborate With Physicians, Nurses, And Other Healthcare Providers In Managing Anticoagulation Therapy For Patients.
