- Collaborate with the healthcare team to develop and implement individualized treatment plans for patients with complex cardiovascular conditions.
- Conduct medication therapy management and ensure appropriate use of medications for patients in the cardiology department.
- Serve as a subject matter expert on cardiology medications and provide guidance to other healthcare professionals.
- Monitor and assess patient response to medication therapy, making recommendations for adjustments as needed.
- Educate patients and their families on proper medication use, potential side effects, and drug interactions.
- Participate in rounds and provide clinical pharmacy services to patients in the cardiology department.
- Conduct drug utilization reviews and ensure compliance with formulary and regulatory guidelines.
- Actively participate in quality improvement initiatives and research projects related to cardiology medications.
- Keep abreast of new developments and evidence-based practices in cardiology pharmacy.
- Maintain accurate and up-to-date patient records and documentation.
- Provide training and mentorship to pharmacy residents and students.
- Support the pharmacy department in maintaining compliance with all accreditation and regulatory standards.
- Collaborate with other healthcare professionals to identify and resolve drug therapy problems.
- Participate in interdisciplinary committees and meetings to improve patient outcomes and promote best practices.
- Act as a liaison between the pharmacy department and the cardiology team to ensure effective communication and coordination of care.
- Maintain a high level of professionalism and adhere to ethical standards in all interactions with patients and healthcare professionals.
- Continuously evaluate and improve clinical pharmacy services within the cardiology department.
- Adhere to all hospital policies and procedures related to medication use and patient care.
Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited Pharmacy Program.
Successful Completion Of Post-Graduate Residency Training In Cardiology Or Equivalent Clinical Experience.
Board Certification In Cardiology Pharmacy (Bccp) From The Board Of Pharmacy Specialties (Bps).
Current State Pharmacy License In Good Standing.
Strong Knowledge And Understanding Of Cardiovascular Diseases, Pharmacotherapy, And Evidence-Based Medicine.
