Cardiology Clinical Pharmacy Specialist

The ideal candidate will be a licensed pharmacist with experience in a cardiology setting, who is knowledgeable of evidence-based practice guidelines and has excellent interpersonal and communication skills. The Cardiology Clinical Pharmacy Specialist will serve as a critical member of our pharmacy team and will play an integral role in providing care to patients with cardiovascular diseases. Required qualifications include:• Bachelor’s degree in Pharmacy, Doctor of Pharmacy, or equivalent from an accredited college of pharmacy• Current licensure as a registered pharmacist in the state of Indiana• Minimum of two (2) years of experience in a clinical pharmacy setting• Knowledge of evidence-based practice guidelines and medication safety• Excellent communication and interpersonal skills• Ability to work collaboratively in a team environment

Responsibilities of a Cardiology Clinical Pharmacy Specialist:

1. Provide expert pharmacological consultation to patients with cardiovascular diseases.
2. Medication order and review, including comprehensive review of patient medication history.
3. Participate in interdisciplinary rounds and provide medication recommendations.
4. Develop and implement evidence-based clinical pharmacy programs and initiatives.
5. Serve as a clinical resource to pharmacy technicians, pharmacists, and other health care providers.
6. Monitor drug therapy regimens for efficacy, safety, and cost-effectiveness.
7. Communicate with patients, families, and other health care professionals as necessary.
8. Participate in the development of medication formularies and therapeutic guidelines.
9. Manage medication supply, inventory, and distribution.
10. Stay up-to-date on new medications and treatments for cardiovascular diseases.
