  1. Serve as a patient advocate and guide for individuals facing breast cancer, offering support and resources throughout their journey.
  2. Use a holistic approach to provide comprehensive care and support to patients, addressing their physical, emotional, and social needs.
  3. Serve as a liaison between patients and healthcare providers, ensuring clear and effective communication.
  4. Conduct comprehensive assessments of patients' needs and develop individualized care plans to address those needs.
  5. Educate patients and their families about breast cancer, its treatments, and available resources.
  6. Collaborate with healthcare team members to coordinate and facilitate appointments, tests, and procedures for patients.
  7. Maintain accurate and up-to-date patient records, ensuring confidentiality and adherence to HIPAA regulations.
  8. Connect patients with community resources and support groups to enhance their care and well-being.
  9. Stay current on the latest advancements and treatments in breast cancer care to provide the best support and guidance to patients.
  10. Provide emotional support and counseling to patients and their families, helping them cope with the challenges of breast cancer.
  11. Monitor and evaluate patients' progress and adjust care plans as needed to ensure optimal outcomes.
  12. Advocate for patients' rights and needs, helping them navigate the healthcare system and overcome any barriers to care.
  13. Collaborate with other healthcare professionals and participate in interdisciplinary team meetings to ensure coordinated and quality care for patients.
  14. Serve as a source of information and support for colleagues and healthcare professionals, sharing knowledge and expertise on breast cancer care.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field: A Breast Center Patient Navigator At Franciscan Health Must Have A Minimum Of A Bachelor's Degree In Nursing Or A Related Field To Be Considered For The Position. This Educational Background Ensures That The Candidate Has A Strong Foundation In Patient Care And Healthcare Knowledge.

  • Experience In Breast Health: A Breast Center Patient Navigator Should Have Previous Experience Working In A Breast Health Setting, Such As A Breast Center Or Oncology Department. This Experience Will Provide The Necessary Knowledge And Understanding Of Breast Health Processes And Procedures.

  • Knowledge Of Breast Cancer Treatment And Support Services: The Ideal Candidate For A Breast Center Patient Navigator Should Have A Thorough Understanding Of Breast Cancer Treatment Options And Support Services Available To Patients. This Will Allow Them To Effectively Guide And Support Patients Through Their Treatment Journey.

  • Excellent Communication And Interpersonal Skills: As A Breast Center Patient Navigator, The Ability To Effectively Communicate With Patients, Their Families, And Healthcare Providers Is Crucial. Strong Interpersonal Skills Are Also Necessary To Build Trust And Provide Emotional Support To Patients During Their Breast Cancer Journey.

  • Strong Organizational And Time Management Skills: Breast Center Patient Navigators Are Responsible For Coordinating And Managing The Care Of Patients, Which Requires Strong Organizational And Time Management Skills. This Includes Scheduling Appointments, Coordinating With Healthcare Providers, And Ensuring Timely Follow-Up Care For Patients.
