
ACO Care Manager/Social Worker

- Coordinate and manage care for patients enrolled in the Accountable Care Organization (ACO).
- Provide comprehensive and compassionate care to patients in need.
- Collaborate with other healthcare professionals to develop and implement care plans for patients.
- Conduct assessments and identify patients' physical, emotional, and social needs.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and goals.
- Monitor and evaluate patient progress and make necessary adjustments to care plans.
- Implement interventions and provide resources to support patients' physical, emotional, and social well-being.
- Document patient interactions, care plans, and progress in a timely and accurate manner.
- Advocate for patients' needs and rights within the healthcare system.
- Stay up-to-date on industry trends and best practices in case management and social work.
- Adhere to all regulatory and ethical standards in patient care.
- Participate in team meetings and case conferences to discuss patient care and outcomes.
- Collaborate with community resources and organizations to provide additional support to patients.
- Maintain confidentiality and privacy of patient information.
- Continuously strive to improve and enhance the quality of care provided to patients.
Bachelor's Or Master's Degree In Social Work Or Related Field.
Minimum Of 2-3 Years Of Experience In Healthcare Or Case Management.
Knowledge Of Care Management Principles And Practices, Including Utilization Management And Population Health Management.
Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With Patients, Families, And Interdisciplinary Teams.
Current Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Of Indiana (Or Ability To Obtain Within A Specified Timeframe).
