Social Worker - Palliative Care

Exeter Hospital is looking for a compassionate and experienced Social Worker to join our Palliative Care team. In this role, you will provide vital support to patients and their families facing a life-limiting illness. We are seeking a special individual who has a strong commitment to providing a high-standard of care and a passion for helping those in need. The ideal candidate for this position will possess a Master's degree in Social Work and experience in working in a palliative care setting. They must be an excellent communicator, possess the ability to empathize with patients and their families, and be able to effectively collaborate with other members of the health care team. Additionally, they must be organized and able to multitask in a fast-paced environment. Responsibilities: • Provide support, guidance, and resources to patients and their families facing a life-limiting illness • Assess patient and family needs and provide individualized care plans • Collaborate with other members of the health care team to ensure best possible outcome for the patient • Provide emotional support and counseling to patients and their families • Develop and maintain relationships with community resources to provide additional support for patients • Document patient progress and interventions • Maintain professional standards of practice and ethical behavior • Work in a fast-paced environment and multitask effectively • Follow up with patients and families to ensure their needs are being met
