Dietitian - Home Care

Exeter Hospital is seeking an experienced and passionate Dietitian – Home Care to join its healthcare team. The successful candidate will be responsible for providing nutritional care to patients in their homes or other community settings. The ideal candidate should have a Bachelor’s Degree in Nutrition or Dietetics, be a Registered Dietitian (RD) with the Commission on Dietetic Registration (CDR), and have at least one year of experience as a Dietitian in a clinical setting. Responsibilities:

1. Provide nutritional care to patients in their homes or other community settings.
2. Evaluate nutritional needs of patients and recommend dietary changes accordingly.
3. Develop and implement individualized nutrition care plans for each patient.
4. Monitor and document patients’ progress and adjust plans accordingly.
5. Educate patients and families on nutrition and ensure that they have the resources to make healthy food choices.
6. Collaborate with other healthcare team members to ensure the best possible care for each patient.
7. Stay up-to-date on the latest nutrition research and trends.
8. Maintain accurate records and documents for each patient.
9. Advocate for patients’ nutritional needs and rights.
