Job Description

Ernest Health is looking for a compassionate Social Worker PRN to join our team and help us provide the highest quality of patient care. This individual must demonstrate commitment to the mission of Ernest Health and possess the qualifications and skills necessary to provide excellent care for our patients. We are looking for someone with a personal touch who can work with individuals, families, and groups to assess their needs and provide counseling and support services. The ideal candidate will be a self-starter with a passion for social work and a desire to make a difference in the lives of our patients and their families.Required Qualifications:• Master’s degree in Social Work from an accredited institution• Advanced knowledge of laws and regulations related to social work practice• Two years of experience in a healthcare setting• Demonstrated ability to work in a fast-paced, team-oriented environment• Excellent oral and written communication skills• Proficiency in computer applications, such as Microsoft Office and electronic medical record systems• Ability to work independently and manage multiple tasks simultaneously• Ability to maintain professional boundaries and confidentiality

Responsibilities:

• Provide patient counseling and support services to individuals, families, and groups • Assess patient needs and develop appropriate treatment plans • Document patient care and progress in the electronic medical record system • Maintain professional boundaries and confidentiality of patient information • Educate patients and families about resources and services available in the community • Develop and maintain collaborative relationships with community partners • Monitor patient care and progress to ensure positive outcomes • Participate in departmental meetings and initiatives • Maintain current knowledge of laws and regulations related to social work practice • Assist with discharge planning and follow up care for patients

Compensation
