  1. Ensure the safe and effective use of medication for patients through accurate and timely dispensing, monitoring, and counseling.
  2. Adhere to all federal and state laws and regulations related to pharmacy practice, including controlled substance laws and regulations.
  3. Collaborate with physicians and other healthcare professionals to develop and implement individualized treatment plans for patients.
  4. Conduct medication regimen reviews to identify potential drug interactions, drug allergies, and inappropriate medication use.
  5. Provide education and counseling to patients and their families on proper medication use, potential side effects, and drug interactions.
  6. Maintain accurate and complete patient records, including medication profiles and documentation of interventions and recommendations.
  7. Participate in medication error reporting and monitoring processes to ensure patient safety.
  8. Stay current with new medications, therapies, and drug-related information through ongoing education and training.
  9. Monitor and manage inventory levels to ensure adequate supply of medications and minimize waste.
  10. Comply with all company policies and procedures, including those related to patient confidentiality and safety.
  11. Demonstrate a commitment to providing exceptional care and service to patients and their families.
  12. Work collaboratively with interdisciplinary teams to ensure the best possible outcomes for patients.
  13. Adhere to all infection control and safety protocols to maintain a safe and healthy work environment.
  14. Maintain a high level of professionalism and positive attitude in all interactions with patients, families, and colleagues.

Job Qualifications
  • Active And Unrestricted Pharmacist License In The State Where The Facility Is Located.

  • Bachelor's Degree In Pharmacy Or Pharmd From An Accredited Institution.

  • Minimum Of 1-2 Years Of Experience In A Hospital Or Long-Term Care Setting.

  • Knowledge Of State And Federal Laws And Regulations Pertaining To Pharmacy Practice.

  • Ability To Work Flexible Hours, Including Weekends And Holidays, And Respond To Emergency Situations As Needed.
