  1. Provide case management services to patients in a compassionate and empathetic manner.
  2. Collaborate with healthcare team to develop and implement individualized care plans for patients.
  3. Conduct thorough assessments of patients' medical, social, and emotional needs.
  4. Coordinate and monitor patient progress, ensuring timely and appropriate interventions.
  5. Communicate effectively with patients, their families, and healthcare team members.
  6. Maintain accurate and up-to-date patient records.
  7. Adhere to all legal and ethical standards in patient care.
  8. Monitor and manage patient discharge planning.
  9. Advocate for patients' needs and rights.
  10. Stay current on healthcare regulations and best practices.
  11. Participate in interdisciplinary team meetings and contribute to treatment planning.
  12. Provide education and support to patients and their families.
  13. Handle crisis situations in a calm and professional manner.
  14. Continuously evaluate and improve processes to enhance patient care.
  15. Maintain confidentiality of patient information.
  16. Work collaboratively with insurance companies and other external stakeholders.
  17. Adhere to all safety and infection control protocols.
  18. Demonstrates a commitment to professional development and ongoing learning.
  19. Participate in quality improvement initiatives.
  20. Other duties as assigned.

Job Qualifications
